- 2X2 ID Picture
- Certificate of Academic Excellence signed by the principal (para sa mga Academic Scholars)
- Brgy. Certificate of Indigency
- Assessment of Fees/Enrollment Form
- Senior HS Report Card o Certificate of Grades signed by the Registrar
- Photocopy of Voter's ID/Certificate o Birth Certificate
