HOSPITAL BACKGROUND
- A Level 1, 50-bed authorized bed capacity hospital
- Owned and operated by the Provincial Government of Isabela;
- Sole government district hospital found on the western side of Isabela;
- The only district hospital in the 5th District of Isabela;
- Serves the municipality of Aurora, San Manuel, Mallig, Quirino, Quezon, Roxas, Burgos, Luna and other parts of Kalinga and Mt. Province.
- A Level I Referral Hospital License to Operate from the Department of Health and accreditation from PhilHealth;
- A one-story building with a floor area of more or less 4,500 square meters in a lot area of more or less 26,896 square meters located along the National Highway of San Antonio, Roxas, Isabela.
- Licensed as Community Isolation Unit Starting December 4, 2020
- With 13 Steel houses with a floor area of 165 square meters each, that accommodate COVID-19 related cases.
MISSION
To readily provide the curative, as well as, preventive health needs of our patients encompassing the major disciplines of medicine: Obstetrics and Gynecology, Pediatrics, Internal Medicine, Surgery, and Anesthesia, that is affordable and free for eligible clients and to assist and navigate patients in availing services not within our institutions’ capacity through the Public Health Unit.
VISION
A 75- Bed, departmentalized, Level-1 Center of Quality Health Care Facility, equipped with complete health services, and diagnostics, along with adequate, competent, and efficient manpower by 2028.

VISION
A Tertiary Hospital providing quality comprehensive Medical Care and Health Service working in close coordination with empowered patient, families and communities.
MISSION
To provide adequate accessible, affordable and acceptable curative preventive and promotive Hospital Care to the people of Isabela and its environs, particulary to the poor.
OBJECTIVES
- To upsize authorized bed capacity from 100 to 200 bed capacity.
- To upgrade accreditation and hospital licensing from level 2 to level 3 category (Tertiary).
GOAL
Quality, accessibility and safety of health facilities and services are ensured.
HOSPITAL HISTORY
The Isabela Provincial Hospital was created by virtue of an act of the Philippine National Assembly in 1939, thereby giving way for the construction of the main building in 1940, neither the completion nor the opening of the hospital for the public service due to outbreak of hostilities.
On August 15, 1945, Dr. Jose Valdez, who was the head of the Philippine Civilian Affairs unit (PCAU), turned over to the government this 1st civilian hospital with all supplies, equipments, medical and nursing staff and other personnel who were providing basic medical and surgical supplies and ward operating and dispensary equipment and started operating as the Isabela Provincial Hospital as general under the Bureau of Health.
Because of budgetary constraints, numbers of personnel were reduced leaving a skeletal force to continue its operation with Dr. Rodolfo B. Silvestre as acting Chief of Hospital 'til March 11, 1946.
On March 12, 1946, Dr. Victorino L. Vergara, was appointed as the 1st Chief of Hospital of Isabela Provincial Hospital and in 1947, government hospital was transferred tot he bureau of Department of Health. In this administration, he was able to tap the support of the incumbent Gov. Faustino N. Dy and gave him 5 units prefab building from the provincial allocation. These were used for the construction of the OR, OB-Gyne buildings that can accommodate about 12 beds.
With the presence of an operating room and the availability of ample medical, surgical supplies and simple equipments, it could already meet the demand for common procedure. Later was the construction of an out-patient department, dental and administrative office with Dr. Vergara as Chief of Hospital until he retired and was replaced by Dr. Jesus Maddela in 1960.
These accomplishments were attributed to Dr. Vergara's genuine desire to develop the hospital and to attain adequate medical and surgical supplies for the use of the different department.
On the other hand, the FOA, MSA, ICAS of the United States provide free hospital beds, cabinets, chairs, tables, generator unit and other technical support which the provincial government could not provide.
Realizing the vital support of the Provincial Government, a new operation and OB-Gyne building that can accommodate 12 beds was constructed and the old one was converted into emergency room and bedrooms for the good of the service. Also, a medicare room for medicare patients.
The hospital started from 25 beds, thereby increasing the bed capacity to 100 in 1969 and as per directive from the office of the Secretary of Health, It became teaching and training hospital.
Dispensary, Pharmacy, Laboratory, X-ray, Dental, Administrative Office and out-patient department was later contracted and this time, bigger and more spacious, to be able to accommodate the increase number of patient daily.
Under the administration of Dr. Maddela, additional improvements were achieved such as construction of ornamental lighting fixtures around the hospital compound and repair of Doctor's quarters. As a result, the hospital was adjudged as the best in Cagayan Valley by the Regional Health Office in Tuguegarao and at the same time the cleanest government hospital in the entire Philippines by the Committee on Clean-up Week in 1964.
Due to continued increase in the occupancy rate, there was a need to decongest the overcrowded rooms, thus the construction of the contagious pavilion in 1973 for patients to be isolated.
Though the hospital is beset with problems in its operation, the management was able to continuously improve its quality of health service and improve the capability of its staff by sending them to training in different fields of specialties like Anesthesia, EENT, Surgery, etc.
By 1998, under the administration of Gov. Benjamin G. Dy, a bigger tap of the line 4 story hospital building was constructed in front of the provincial hospital building. Early 2000, the Isabela Provincial Hospital was relocated to the new hospital building and was renamed Gov. Faustino N. Dy, Sr. Memorial Hospital under SP. Resolution No. 0168.
At present, the Gov. Faustino N. Dy, Sr. Memorial Hospital (GFNDMH) is Level II PHIC accredited hospital.
MGA TUNGKULIN NG PASYENTE
- Magbigay ng tama at kumpletong impormasyon tungkol sa karamdaman, mga nakaraang sakit, pagkaospital, pag-gagamot at iba pang bagay na nauugnay sa inyong pagkakasakit.
- Makipagtulungan sa inyong Doktor o Tagapangalaga tungkol sa paraan ng pang-gagamot sa inyo.
- Alamin o unawain ng inyong karamdaman at ang pang-gagamot na ginagawa sa iyo o 'di kaya manghingi ng karagdagang paliwanag upang lubos na maunawaan it.
- Akuin ang anumang puwedeng mangyari sa iyo sa hindi pagsunod sa ipinayo ng inyong manggagamot.
- Ipaalam sa inyong mga tagapangalaga ang anumang nais o reklamo upang matugunan ang mga ito sa tamang oras.
- Maging magalang sa ibang pasyente at pag-ingatan ang pag-aati ng Ospital.
- Maging magalang sa iyong mga manggagamot at Nars o mage tagapangalaga tulad ng nais mong gawin sa iyo.
- Tugunan sa takdang oras ang anumang bayarin sa Ospital.
PARAAN SA PAG-UUWI NG PASYENTE
- Magtanong ang asawa o kamag-anak sa Doctor, Nurse o Midwife kung maaari ng makauwi ang pasyente.
- Kung maaari ng makauwi ang pasyente, hingiin sa Nurse o Midwife ang "CLEARANCE".
- Ipapirma ang Clearance sa mga iba't ibang Departamento na nakasulat dito.
- Sa mga kamag-anak ng pasyente na may dalang sertipikasyon ng walang kakayahang magbayad o kakulangan sa pagbayad, dalhin ito sa "SOCIAL WORKER" para sa mga ilang katanungan.
- Matapos madala sa social worker, dalhin ang papel sa hepe ng Ospital para maaprubahan ang paghingi ng tulong o diskwento.
- Sa mga pasyenteng may babayaran, magpunta sa kahera upang makapagbayad. Hingiin ang resibo sa kahera.
- Kunin ang GATE Pass sa BILLING SECTION at iwan ang "STATEMENT OF BILLING" dito.
- Muli ay magpunta sa Nurse o Midwife upang ibigay ang Clearance.
- Ibigay o iwanan ang gown at bedsheet sa Nurse o Midwife at papirmahan ang gate pass.
- Hintayin ang paglabas ng pasyente. Siguraduhing dala ng kamag-anak ang gate pass.
- Ipakita ang gate pass sa guwardiya bago umalis.
- Laging magpasalamat sa mga taong nagsilbi sa inyo sa loob ng Ospital.
